IN HOUSE PLAN (Patients with no dental insurance)
We offer a dental plan of our own that has no maximum, no deductible, no preauthorizations, no waiting period and no pre existing conditions.
In an effort to help our patients without dental insurance, we have designed an affordable alternative. This plan is not dental insurance and is exclusive to our office. Our In-House Plan will allow you the opportunity to visit our office regularly for your preventative care. In addition, your restorative work if needed, is also discounted to help you in these difficult economic times. This plan was implemented to help patients continue regular visits to the office. Preventative care (cleanings) is the cornerstone to long-term optimal oral health. We sincerely hope this plan provides you with a viable option for quality dental care. Coverage is in effect for one year, which begins when membership has been paid in full. Members will be notified one month prior to expiration of in house benefits. Appointment times are limited, and it is the responsibility of the member to schedule appropriate visits.
In House coverage includes: 2 Regular dental check ups, Exam, Dental Cleaning, Oral Hygiene Instruction, Periodontal Evaluation, Digital x-rays (If necessary), Oral Cancer Screening, TMJ Screening, Perio Scale & Root Planing 25% discount, Perio Maintenance x2 (2 at 25% discount), Unlimited Emergency visits. 25% Discount off our usual and customary fee: The 25% discount applies to fillings, crowns, bridges, extractions, partials and dentures. Discount EXCLUDES Whitening, Botox, Dermal Filler and Oral Appliance. When signing up for the In House you will get $100 towards your first restorative appointment.
The following services are covered at 100%: Comprehensive Exam (New patient/initial visit), Periodic Exam, Limited/Emergency Exam, Oral Cancer Screening, Bitewing x-rays, Periapical x-rays, Panorex x-rays, Complete Series, Preventive Sealants to age 16, Child Prophylaxis , Fluoride to age 18.
Membership in our in-house dental benefits plan is available to all patients without dental insurance coverage. Annual fee must be paid in full to activate membership. Membership is not transferable or refundable.
